Physical/Drug Test Consent Forms
All students participating in athletic activities MUST have a physical, medical release and drug test consent form on file in the Athletic Office BEFORE they attend any practices. Physicals must be dated after May 1, 2009 in order to be valid for the 2009-10 school year.
